The present system of health care in India is not capable of meeting the specific needs of its ageing population. The system, characterised by loose groups of physicians, hospitals, and healthcare institutions operating in a highly dispersed manner without access to complete information on the patients’ condition and medical history, lacks the ability to address chronic ailments associated with old age. The situation becomes worse with the need to address multiple chronic conditions typically associated with the ageing population.

Life Circle model is centred around a trained nurse supported by family doctor and aided by systems and processes required for rigorous and regular monitoring and follow up of an enlisted clientèle of senior citizens and bed ridden. The core of the model is a comprehensive health assessment at home for each senior citizen. This is to be done by the nurse supported and guided by the family doctor. The nurse is to work with the senior citizen, the care giver, and the family doctor to customise an evidence-based comprehensive health plan.
